Some hospital discharge teams don’t fully explore IRF eligibility. I advocate for appropriate IRF placement and refer families when the clinical picture supports it.
Inpatient Rehabilitation Facilities receive the majority of their referrals through hospital discharge planning teams. However, families who contact me after a stroke, hip fracture, brain injury, or major surgery sometimes haven’t been fully evaluated for IRF eligibility — or have been defaulted to SNF without an IRF conversation. I advocate for appropriate IRF placement when it’s clinically justified, and I build referral relationships with Texas IRFs so I can make accurate, informed referrals when the opportunity arises.

How It Works
Family contacts me post-hospitalization
A family or hospital discharge planner reaches out while the patient is still in the hospital. I gather clinical information to assess IRF eligibility and appropriateness.
IRF eligibility assessment
I evaluate whether the patient meets IRF criteria — qualifying diagnosis, therapy tolerance, rehab goals, and insurance authorization potential. If IRF is appropriate, I contact partner IRFs.
Warm referral with clinical brief
I make a warm introduction to your admissions team with a brief on the patient’s situation, goals, and family contacts. This reduces the time between referral and admission decision.
Family coordination and follow-through
I help the family understand the IRF admission process, what to expect, and how to navigate insurance authorization. I stay available through the admission to reduce friction.
