I come to the hospital bedside or meet virtually with the family, conduct a detailed intake, and take over the placement coordination process while keeping you informed.

I handle facility research and tours

I research and contact appropriate facilities, arrange tours, coordinate with admissions teams, and guide the family through their decision — all on a timeline compatible with your discharge date.

Placement is confirmed, you are updated

Once placement is confirmed, I notify your discharge planning team so you can complete your documentation. I remain the family’s point of contact through move-in.
