Hospital discharge planners work under enormous time pressure — the average hospital stay is under five days, and discharge teams are managing dozens of cases simultaneously. When a family needs post-acute placement and requires more than the 20–30 minutes a discharge planner can realistically dedicate to their options, a placement specialist serves as an extension of the discharge planning process. I am available to meet with families during the hospital stay, explain their options, tour facilities with them, and handle the logistics of post-acute placement — letting the discharge planner focus on what only they can do.

You identify a family that needs placement support
When a family needs more guidance on post-acute options than your team can provide — Medicaid cases, complex needs, out-of-town family, family disagreement — you refer them to me.
I meet with the family
I come to the hospital bedside or meet virtually with the family, conduct a detailed intake, and take over the placement coordination process while keeping you informed.
I handle facility research and tours
I research and contact appropriate facilities, arrange tours, coordinate with admissions teams, and guide the family through their decision — all on a timeline compatible with your discharge date.
Placement is confirmed, you are updated
Once placement is confirmed, I notify your discharge planning team so you can complete your documentation. I remain the family’s point of contact through move-in.
