Hospice agencies and senior placement specialists intersect in important ways. Many families who contact me for placement have a loved one approaching the end of life — and a hospice conversation needs to happen alongside or before the placement conversation. Conversely, hospice agencies often have clients at home whose family can no longer manage the caregiving demands, and who need placement into an assisted living, memory care, or SNF setting where hospice can continue. If a family contacts me and a hospice conversation is clearly needed, I make a warm referral to a trusted hospice agency. If a hospice client can no longer be cared for at home, the hospice team refers them to me.

For families who need placement while on or about to start hospice, I identify partner communities with strong hospice integration — so the family doesn’t have to navigate two organizations independently.
