Geriatric care managers (GCMs) — also called aging life care specialists — provide ongoing assessment, care coordination, and advocacy for older adults and their families. When a client in their caseload needs placement, a GCM may not have the time or the current facility knowledge to navigate the placement process in depth. A referral to a placement specialist — or a collaborative arrangement where the GCM maintains the care management relationship and the specialist handles placement logistics — serves the client better than either professional working in isolation.
