Person-Centered Care

Person-centered care is a care philosophy that prioritizes each resident’s individual preferences, history, values, and autonomy — adapting care around the person rather than fitting the person into institutional routines.

Full Definition

Person-centered care (also called resident-centered care or patient-centered care) is a philosophy and model of care delivery that places the individual’s preferences, values, life history, relationships, and goals at the center of care decisions — rather than fitting the person into a standardized institutional routine.

In practice, person-centered care looks like: knowing what a resident prefers for breakfast, not just what the facility serves; adapting wake and sleep schedules to individual patterns rather than institutional shift changes; involving residents in decisions about their daily activities; honoring cultural, religious, and personal traditions; and supporting meaningful relationships and activities even as function declines.

Research consistently shows that person-centered care is associated with improved quality of life, reduced behavioral symptoms in dementia, lower use of antipsychotic medications, higher staff satisfaction and lower turnover, and better family satisfaction.

For families evaluating Texas senior care communities, person-centered care is evident in observable behaviors during a tour: Do staff know residents by name and preference? Do residents appear engaged or are they sitting inactive? Are activities being conducted or just offered? Is the community’s culture warm and relationship-focused, or task-focused and transactional? Person-centered care culture cannot be assessed by reading a brochure — it requires a visit, preferably unannounced.
