Medicare Three-Day Rule

The Medicare Three-Day Rule requires a qualifying inpatient hospital stay of at least 3 consecutive days before Medicare Part A will cover skilled nursing facility care — observation days do not count toward this threshold.

Full Definition

This rule is formally called the “3-day qualifying hospital stay” requirement for Medicare Part A SNF coverage. The patient must have been formally admitted as a hospital inpatient (not placed under observation status) for at least 3 consecutive calendar days (not counting the discharge day).

Medicare Advantage plans are technically permitted to waive this requirement, though in practice many still apply it or have equivalent authorization requirements.

For families planning post-acute care: confirm inpatient status at admission, not on the day before discharge. If a family member is admitted to the hospital for a condition that may require post-hospital SNF care (hip fracture, stroke, pneumonia), asking the case manager on day one whether the patient is in inpatient status prevents the surprise of denied SNF coverage at discharge.
