Restorative Nursing

Restorative nursing is a program in skilled nursing facilities that helps residents maintain functional abilities gained during skilled therapy through ongoing daily practice with trained nursing staff.

Restorative nursing is a program in skilled nursing facilities designed to maintain and reinforce functional abilities that residents have gained during skilled physical, occupational, or speech therapy — after the skilled therapy episode has concluded. It bridges the gap between intensive skilled rehabilitation and the long-term maintenance of function.

Restorative nursing activities are carried out by nursing assistants trained in restorative techniques under the supervision of a licensed nurse. Activities include: range-of-motion exercises, walking programs, ADL training reinforcement, splinting and positioning programs, and communication maintenance activities for residents with aphasia or dementia.

Restorative nursing is not the same as skilled therapy and does not require a physician’s order in the same way — it is part of the facility’s care planning for long-term maintenance. CMS surveys evaluate whether facilities have active restorative nursing programs and whether they are effective.

For families, restorative nursing is important because it determines whether the gains achieved during post-acute rehabilitation are maintained over time. A facility that provides good short-term rehabilitation but lacks effective restorative nursing may see residents decline back to pre-admission levels within months. When evaluating facilities, asking about restorative nursing programs and whether restorative activities are actually documented and occurring is a meaningful quality question.
